If you receive Support at Home funding your Neighbourhood team includes a Care Partner — your go-to person for planning, coordination and support for your day-to-day care.

You’ll have regular contact with your Care Partner, who will check in with you, listen to what’s important to you, and help make adjustments to your support as needed. Whether it’s organising services, navigating changes, or simply being there when you need advice, your Care Partner is with you every step of the way.

And because your care partner is based in your local Neighbourhood, they understand the area you live in — from community groups and local services to the practicalities of getting around.

With your Care Partner, you’ll have the confidence of knowing your care is coordinated by people who are genuinely on your side — and always just a phone call away.
