What is Hospital in the Home?

Hospital in the Home (HITH) is a program that provides professional, short-term, acute treatment in the comfort and privacy of home, for people who would otherwise need to be in hospital.

Working alongside your hospital medical team, BlueCare develops an in-home treatment plan that is safe and suits a patient’s needs.

Blue Care Hospital in the Home services are currently available from:

• The Wesley Hospital
• St Andrew’s War Memorial Hospital
• Buderim Private Hospital

How Hospital in the Home works

The hospital medical team determines if a patient is eligible to receive treatment safely at home. The BlueCare Hospital in the Home team will liaise with appropriate medical staff to develop a personalised treatment and recovery plan. Arrangements are then made for the patient to transfer home and continue their treatment.

Patients will receive in-home visits from our friendly and professional BlueCare nurses. The frequency will depend on the condition and treatment plan and will be discussed with the patient before they transfer home from hospital. Their progress is monitored carefully and in accordance with their medical treatment plan.

Our team will:

• Schedule medical reviews and facilitate clinic appointments.
• Provide advice on what to do in an emergency.
• Provide with after-hours telephone support.
• Develop a discharge plan with the patient.
• Facilitate follow-up services with appropriate providers as required.
• Advise the patient’s GP of their Hospital in the Home treatment.
