From 1 November 2025, aged care in Australia is evolving. The new Support at Home program will replace the current Home Care Packages, offering a more flexible and personalised way to help you live well at home. Designed to be easier to access and navigate, these changes aim to improve the quality of care for older Australians. Support at Home is designed to help you stay independent and live safely in your own home. From 1 November 2025, if you're on a Home Care Package, you'll automatically transition to this new program.
The services you can access are grouped into three categories:

Support at home servicesIf you are new to aged care after 1 July 2025, you'll be supported through the Single Assessment System, a simpler, more consistent way to access government-funded aged care.
Once assessed you will be assigned a funding level and personalised care plan. Care services will be grouped into three categories:

Grandfathered and non-grandfathered clients
The Australian Government will be sending letters to all home care package clients about your funding and contribution arrangements under Support at Home. Which letter you receive is dependent on when your Home Care Package (HCP) was approved:
If you were approved for a Home Care Package on or before 12 September 2024 you will transition to Support at home as a grandfathered client.
