Accessing Blue Care Services

With over 250 centres, please contact your nearest Blue Care centre or phone Blue Care Head Office on 07 3377 3377 for more information and how to access our services.

Referral to Blue Care services may come from:

  • yourself
  • friends 
  • family members
  • carers
  • hospitals
  • general practitioners
  • health agencies
  • Aged Care Assessment Team.

Please note for some treatments such as wound dressings or physiotherapy, a written referral from a doctor may be needed.

 

Who we care for...

Blue Care provides care to all members of the community including:

  • frail aged
  • people with a disability
  • those requiring nursing or allied health support following release from hospital
  • individuals and carers who are in need of support and education.

Regardless of:

  • financial circumstances
  • cultural background
  • spiritual background.

Respect for individuals’ personal values, beliefs, and culture is very important to us.

 

Eligibility criteria

Most Blue Care services are provided within government funded community care programs, and each program has regulations about who is entitled to receive services. 

For many programs your eligibility for services can be assessed over the phone.

Priority for service

We try our best to provide you with the services you require as fast as possible,  however sometimes it may be necessary for you to be placed on a waiting list.

Please be assured that Blue Care prioritises services to people in accordance with urgency of need as well as resource availability (funds and staff).

 

Assessment of need

If you are likely to require a number of different community services, a comprehensive assessment may be necessary. 

It is important to determine all of your care/social needs (e.g. transport, assistance with shopping, meals, personal care, medication assistance).

Your needs may be ongoing or temporary, and so discharge planning can form part of the assessment.  
Blue Care comprehensive assessments:

  • are person focussed
  • include client, family, and carer participation in the assessment process
  • complement existing resources and strengths
  • lead to individualised care/support solutions
  • support coordinated care for clients in their homes.

To enable informed client choices, Blue Care will provide and explain all relevant information throughout the assessment process.

 

Service provision

Blue Care provides most community care services using its own staff and facilities.  However Blue Care will refer clients to other agencies for some services if required.  Case management and care coordination services are provided.
