Aged Care Packages (CACPs)
Community Aged Care Packages (CACPs) focus on allowing older people
to remain in their homes. This ensures minimal disruption to their
life and may prevent entry into low level residential care facilities
such as a hostel.
Before a CACP can commence individuals must undergo an assessment
by the Aged Care Assessment Team (ACAT) to establish how physical
and psychological independence can be achieved or maintained. This
is important for both the client and their family.
Each client's needs are different and can change over time. Therefore
care packages vary from client to client and the services are designed
to accommodate changing needs.
Blue Care can provide assistance with:
- personal care (bathing and dressing)
- preparation of meals, including special dietary needs
- light housework
- laundry
- monitoring of medications
- transportation
- social activities
- shopping
- access to advocacy
- access to Allied Health staff
(eg. Physiotherapist, Podiatrist,
Occupational Therapist).
Services take into account lifestyle, cultural background, and religious
beliefs. Interpreter services can be arranged if necessary.
Although CACPs are subsidised by the Commonwealth Government, recipients
are asked to pay a minimal fee for the service.
Please note that special arrangements can be organised in situations
of financial hardship.
