Referrers and Service Providers

Referrals to our services come from a variety of avenues including:

  • self or family
  • hospitals
  • general practitioners
  • community health centres
  • community service providers
  • an Aged Care Assessment Team (ACAT).

How do I make a referral to Blue Care services?

You can refer by telephone or fax. All telephone referrals must be followed up by forwarding a written referral clearly outlining the care requested, together with the client's demographic and care history details.

How quick is the response time for referrals?

Your referral may be answered in person or you may have to leave a message. We will respond to your telephone call or fax within 24 hours of receipt, Monday to Friday. Clients will be contacted and arrangements will be made at a mutually-agreeable time to visit.

What services does Blue Care provide?

We provide an extensive range of care and support services through our community and residential care.

Who does Blue Care care for?

We care for all members of the community including people who are aged or frail and people with a disability. To access services through government-subsidised care programs however, clients will need to be assessed against various eligibility criteria.

How do I know where the nearest Blue Care centres are located?

Search for Blue Care centres by location, proximity to a specific address or available services. There are more than 260 centres and facilities located in over 80 communities throughout Queensland and Northern New South Wales.
